Permission Forms
Forms are found under the main menu on the left - option Forms.
Permission Forms can be customised to be any type of form you would like parents to complete online. Once the forms are created, you can manage the students that will need to complete these forms, email parents to complete them, send reminders to parents that need to complete the forms, export the information from the forms.
These forms can include any of the following:
- Band Camp Forms
- Performance Permission Forms
- Medical Forms
- Band Workshop Forms
Multiple forms can be created and managed at the same time.
A list of all forms that have been created can be seen along with the number of students that have been assigned to that form and how many have completed the form so far.
Creating a New Form
Forms can be created from a blank form or from a template. Several templates have been added to the system so that you can have a starting point and then adjust the form as needed.
Click on 'Add Form' to create a new form. Give your form a name and either choose Blank Form to start from scratch or click on the type of form template in the dropdown list to start with a basic form that you can adjust to what you need.
Once you click the 'Add', you will see the form has been added to the list. You can then click on the form to go into the form to work on creating and managing the form.

Click the 'Edit Form' button to go into the form creation section.

You can update the name of the form or delete the form using the top section.
The tickbox for 'Form is active on portal' is used to put the form onto the parent's portal so that they can complete the form. Once you have completed the form creation and added the students to the list of student's for this form, you can tick this to activate the form for the parents to start completing the form.
Types of Items that can be Added to the Form
There are several different types of items that can be added to a form:
- Heading - this is a heading line for the form and can have a line under the heading as an option
- Text Block - this is a block of information
- Selection List - this is a question that has a list of answers you can customise to any answers you would like. This question has the option to have either single or multiple answers selected by the parents. Each questions can be optional or forced to be answered and has the option for a comment to be allowed per question.
- Comment Question - this is a question allowing the parent to answer a question in the form of a comment only
- Checkbox - this is a question that has a tickbox before the questions allowing the parent to either tick or untick the box. All checkbox questions are listed on the table of information about the students who are on the list and the Grid Heading is the name of the column that will be displayed on the grid.
Adding Items to the Form
You can add each item as you need to by clicking on the item and then filling in the details of the item. This will add the item with the information onto the form.
At anytime, you can click on the item and update the details of the item.
Moving Items around on the Form
If you would like to re-arrange the order of the form items, you can click and hold on any item and you will see the item line zoom in a little bit. Once you see this, you can move the line item up or down the list to move the item to a different part of the form.
Previewing the Form
At any time, you can click the 'Preview' button to view what the form will look like and then adjust the form as you need.
Once you have added in all the form items you want, your form will be ready to send to parents.
When the form becomes set and not updatable
Once you have sent the form out to the parents and you have had any forms completed, you will no longer be able to update the form. This is to avoid the possibility of a questions or information being updated after a form has already been completed by a parent and therefore the answers might not be in line with the questions or information anymore.
If you need to update the information or questions, you will need to clear the information provided by parents and then update the form and then ask the parents to complete the form again.
